Terminology
There is quite some confusion about terminology.
In my view, EHR (Electronic Health Record) should be the envelopping term, including EMR, ECR and PHR.
- EMR (Electronic Medical Record); being all the medical information, either from your General Practitioner or your hospital clinician
- ECR (Electronic Care Record); being all the information from your caregivers (nurses, friends, family), concerning issues like nursing (especially at home), social arrangements (who is bringing me to the hospital), monitoring (how did my bloodpressure, sugar, etc. read)
- PHR (Personal Health Record); being all the information coming from the patient, like his history, his personal observations.
